Evaluations for any of these possible diagnoses are typically covered under your group health policy, or by your employee assistance program. Feel free to call or email us, and we will probably be able to tell you if we accept your plan. If we cannot, we will tell you how to find out. If we do not, we will be happy to give you names of other professionals in your area who may be able to assist.

 

 

 

 

Do You Have a Substance or Alcohol Problem?

  1. In the past year, have you ever drunk or used drugs more than you meant to? Or, have you spent more time using or drinking than you meant to?

  2. Have you ever neglected some of your usual responsibilities because of your usage of alcohol or drugs?

  3. Have you felt you wanted or needed to cut down on your usage in the last year?

  4. Has anyone objected to your drinking or drug use?

  5. Have you ever found yourself preoccupied with wanting to use alcohol or drugs?

  6. Have you ever used alcohol or drugs to relieve emotional discomfort such as sadness, anger, or boredom?

If you answered yes to two or more questions, you may be considered an abuser of, or dependent upon, alcohol or drugs. Four or more positive responses indicates dependence on alcohol or drugs. If you have concerns, schedule an assessment with a professional near you. If you are in our area, we will be happy to assess your needs.


 

 

 

 

Do you suffer from depression?

  1. Have you lost interest in things you used to enjoy?

  2. Are you crying more than usual?

  3. Do you feel worthless at times? Do you feel guilty much of the time?

  4. Have your sleeping and/or eating habits changed recently?

  5. Do you have vivid dreams or nightmares?

  6. Are you having more trouble than usual completing daily chores and responsibilities?

  7. Do you sometimes consider harming yourself, or ending your life?

If you responded with a “yes” to at least two of the first six items, or to number seven, you are suffering from clinical depression. Seek professional help for a complete assessment. In the Plano area, we will be pleased to provide those services.


 

 

 

 

Do You Suffer With Anxiety?

  1. Do you have spells when your mind races, and you cannot stop thinking?

  2. Are there times when your heart races and you feel your chest tightening?

  3. Do you feel closed in when in small areas or crowded rooms?

  4. Do you have periods when you feel disorganized and overwhelmed?

  5. Do you feel “doomed” at times, and think the thoughts will never cease?

  6. Do you feel like everyone is watching you?

Two or more positive responses may indicate clinical anxiety, and an assessment may be in order. Consider seeing a professional for an in-depth evaluation. Again, if you reside or work in our area, we will be pleased to provide those services.
